Responsibilities:
- Oversee CHW activities to ensure effective family engagement and care coordination.
- Monitor performance metrics related to onboarding, service continuity, and waitlist management.
- Lead, mentor, and develop a remote team of CHWs through coaching and supervision.
Qualifications:
- Minimum of 3 years experience as a CHW or similar role.
- Active social work license (LSW, LMSW, or LCSW per NM licensure).
- Bilingual in English and Spanish.
Compensation & Benefits:
- Salary range $65,000 - $80,000 USD per year.
