Responsibilities:
- Lead daily operations and provide direction to Care Coordinators, ensuring effective care transitions and provider connections.
- Coach and develop team members, conduct performance reviews, and deliver training on workflows and program expectations.
- Monitor performance, resolve escalated issues, and drive process improvements to enhance member experience.
Requirements:
- At least 4 years of experience in care coordination, case management, or healthcare services.
- At least 2 years of supervisory or people management experience in a healthcare environment.
- Strong understanding of HIPAA, privacy standards, and culturally responsive service delivery.
Benefits:
