Key Responsibilities:
- Gather complete medication histories and reconcile medication lists.
- Identify adverse drug reactions and adherence gaps, contacting providers as needed.
- Document clinical activities and serve as a consultation resource.
Qualifications:
- Doctor of Pharmacy (PharmD) degree preferred; completion of residency or board certification a plus.
- Current unrestricted pharmacist license in New Hampshire and ability to obtain multi-state licenses.
- Strong communication skills and customer-service focus.
