Responsibilities:
- Evaluate pharmacotherapy for high-risk patients with chronic conditions like diabetes and hypertension.
- Execute clinical protocols and optimize drug coverage benefits for cost-effective care.
- Participate in high-risk rounds and document activities in EMR systems.
Qualifications:
- Active Pharmacist license and Pharm.D. with at least 2 years of experience.
- Intermediate Microsoft Office skills and willingness to obtain additional state licenses.
- Preferred: Ambulatory care experience, board certification, and multilingual skills.
