Therapy Management Clinical Pharmacist (NY Licensed)

Job Summary:

• The Therapy Management Clinical Pharmacist supports the site TM program by providing pharmaceutical care services.
• Works remotely and collaborates with healthcare professionals to ensure safe and effective medication therapies.
• Performs phone-based assessments on disease state management and drug therapy education.

Job Duties:

• Review and evaluate medication orders for accuracy, appropriateness, and safety.
• Provide medication counseling to patients, addressing questions and concerns.
• Collaborate with healthcare providers to develop and monitor medication therapy plans.

Experience and Education:

• Must hold a Doctor of Pharmacy (PharmD) degree and be a licensed pharmacist in New York.
• Strong clinical knowledge relevant to diabetes mellitus and/or weight management.
• Solid computer skills including proficiency in Microsoft Office tools and web-based platforms.
