Community Health Worker, Patient Engagement

Who We Are:

• Imagine Pediatrics is a tech-enabled, pediatrician-led medical group reimagining care for children with special health care needs.
• We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans.

What You'll Do:

• Serve as a community-based, patient-facing resource, building relationships with patients and caregivers through direct, in-person re-engagement.
• Provide hands-on support including program re-enrollment, onboarding, and mobile app setup.
• Educate patients and caregivers on services and community resources, and partner with interdisciplinary teams to address barriers to care.

What You Bring:

• A valid driver's license, a bachelor's degree, and 3+ years of experience in a patient-facing healthcare role.
• Ability to navigate ambiguity, think critically, and manage multiple tech platforms.
• National Community Health Worker certification and bilingual (Spanish) highly preferred.
