Resource Pediatric Registered Nurse

About the Role:

• Serve as a key clinical partner within the Patient Engagement team, supporting re-engagement of previously enrolled patients and families.
• Act as a primary point of contact during re-engagement, delivering timely, family-centered clinical support through virtual touchpoints.
• Ensure patients are quickly connected to appropriate care pathways.

Key Responsibilities:

• Perform comprehensive assessments of patients’ clinical, psychosocial, discharge planning, and financial needs.
• Coordinate with Resource specialists, Behavioral Health, and other interdisciplinary partners to address complex needs.
• Meet established monthly re-engagement productivity and engagement goals.

Qualifications:

• Licensed RN in at least one state with eligibility to register for other state licensures.
• Minimum 1 year care coordination or case management required.
• Bilingual Spanish is required.
