Community Partnerships Manager (Bay Area)

The Community Partnerships Manager is responsible for developing and sustaining strategic relationships with local organizations in the California Bay Area and the Central Valley that serve our addressable population. This role is focused on establishing a strong referral network by creating mutually beneficial partnerships with hospitals, community-based organizations, shelters, and other service providers. The Community Partnerships Manager, Bay Area will identify, develop, and manage relationships with key patient referral sources, including local hospitals, community based organizations (CBOs), shelters, clinics, and other relevant institutions.
