Nurse Practitioner / Physician Assistant

This role involves approximately 70% Telehealth patient care and 30% administrative inbox management using eClinicalWorks (eCW). Responsibilities include conducting telehealth assessments, diagnosing and managing acute/chronic conditions, developing treatment plans, and documenting patient encounters in the EHR. The role also includes managing administrative tasks within eCW, triaging patient messages, and renewing prescriptions based on protocols. The position requires strong communication skills, independent clinical judgment, and the ability to collaborate with a multidisciplinary team to provide integrated healthcare. Requires knowledge of current evidence-based guidelines and standards of care.
