Pt Connection Triage RN-PRN

Role Overview:

• Under the supervision of the RN Patient Scheduler Manager, provides timely, efficient, and accurate triage response and documentation.
• Evaluates patient needs and performs triage within the contact center setting, operating within the scope of nursing practice.

Key Responsibilities:

• Maintains accurate and concise documentation within medical records, ensuring compliance with all guidelines.
• Schedules patients for visits and provides education on patient care, medications, preventative services, diet, and exercise.
• Acts as a clinical resource to all non-clinical connection center personnel.

Qualifications:

• Graduate of an accredited school of nursing and current RN licensure in Georgia or multistate.
• Three years of professional nursing experience, preferably in ER, critical care, or cardiovascular settings; phone triage experience preferred.
• Computer literacy with EPIC and EMR plus ability to multitask in a fast-paced environment.
