Your Role:
- Perform monthly CCM touchpoints and structured symptom assessments for assigned patient panels.
- Escalate urgent issues based on defined triage pathways and collaborate with provider teams.
- Document all patient interactions clearly in EHR and CCM platforms, ensuring compliance.
Compensation & Benefits:
- Competitive annual salary range of $55,000-$65,000 plus quarterly bonus eligibility.
