About the Role:
- You will evaluate, diagnose, and manage chronic kidney diseases through a fully remote platform.
- You will conduct follow-up visits and longitudinal care for established patients.
- You will collaborate with specialists and care team members to ensure coordinated care.
What You'll Do:
- Document all patient encounters accurately and timely within the EMR system.
- Order and interpret laboratory and diagnostic studies.
- Provide patient education on lifestyle, medication adherence, and disease management.
Qualifications:
- You must hold a CA Licensed NP or PA in good standing.
- You need at least 2 years of Nephrology experience in outpatient or telehealth settings.
- You should have strong communication skills and be comfortable practicing autonomously remotely.
