Role Overview:

  • You support care delivery within the Evergreen Aligned team by partnering with Advanced Practice Providers to execute care plans.
  • You focus on enhancing clinical outcomes through patient education, remote monitoring, and medication management.
  • You coordinate care across multidisciplinary teams to ensure patient-centered, holistic support.

Responsibilities:

  • Conduct pre-visit planning, care plan execution, and disease management education for conditions like CKD, hypertension, diabetes, and CHF.
  • Perform medication reconciliation, monitor quality measures, and support remote patient monitoring.
  • Build patient relationships through empathetic communication and self-management education.

Qualifications:

  • Active, unrestricted RN license and 2+ years of clinical experience in chronic disease management.
  • Strong patient education and communication skills, with experience in EHR and data interpretation.
  • Ability to manage multiple patients in a fast-paced virtual environment with strong organizational skills.

Compensation and Benefits:

  • Base salary range is $90,000 to $103,000 annually, with eligibility for quarterly bonus.
  • Benefits include paid time off starting at 4 weeks, 12 paid holidays, medical, dental, vision, life insurance, HSA, 401(k) with match, and paid parental leave.
