About the Role:
- As a Solace Physician, you'll build relationships and facilitate an advocacy experience for patients.
- You'll collaborate with healthcare advocates to help patients achieve health goals and develop a new patient care program.
What You'll Do:
- Learn Solace systems, tools, technology, partners, and expectations, providing clinical expertise.
- Identify and prioritize patients' needs and maintain a streamlined care continuum.
- Oversee patient care plans in partnership with healthcare advocates to address social determinants of health.
What You Bring to the Table:
- Board certification in family medicine, internal medicine, or other relevant specialty.
- Strong clinical and organizational skills, with superior communication and empathy.
- Technical savvy, extreme bias toward action, and embrace change and iteration.
