The Role:
- Work remotely with Medicare patients throughout their healthcare journey.
- Help patients overcome barriers to care and access the resources they need to thrive.
What You'll Do:
- Build strong, trusting relationships with older adults, using listening and empathy.
- Develop patient care plans addressing social determinants of health.
- Support patients through warm hand-offs, weekly follow-ups, and hands-on assistance.
What You Bring:
- Background in social work, community health work, or case management.
- 2+ years of experience in care coordination, patient advocacy, or healthcare navigation.
- Tech-savvy with a reliable computer, high-speed internet, and phone.
