Nurse Care Coordinator Lead (RN)

Leadership Responsibilities:

• Recruit, hire, train, and onboard new team members to build a high-performing care team.
• Provide direct supervision, conduct performance reviews, and coach staff for professional growth.
• Identify developmental needs and facilitate ongoing training and continuing education.

Operational Management:

• Maintain compliance with healthcare regulations including HIPAA and state nursing board requirements.
• Ensure timely documentation and participate in quality improvement and process optimization initiatives.
• Track key performance indicators and coordinate with other departments for seamless patient care.

Communication & Collaboration:

• Serve as a liaison between clinical staff and management, effectively communicating with patients and families.
• Participate in team meetings and clinical huddles to coordinate care and address concerns.
• Collaborate with community partners and external resources to support comprehensive care.
