Key Responsibilities:
- Manage a defined remote patient monitoring and/or chronic care management patient panel, meeting or exceeding individual production standards.
- Direct the performance of your assigned team, ensuring daily productivity, documentation accuracy, and program compliance.
- Serve as the first point of escalation for clinical quality concerns and lead client-facing engagements.
Required Qualifications:
- Bachelor's degree in nursing and active RN license in good standing.
- Minimum 3-5 years of clinical experience with background in care management or chronic disease management.
- Prior lead or supervisory clinical role with direct reports and individual caseload.
Preferred Qualifications:
- Familiarity with remote physiological monitoring, EMR/EHR systems, and patient engagement tools.
- Experience with Federally Qualified Health Centers or value-based care organizations.
- Verbal and written proficiency in Spanish.
