Member Engagement:
- Assist with outreach, engagement, and continuous support for a cohort of members.
- Provide regular check-ins and personalized care.
- Develop and coordinate community resources.
Care Coordination:
- Assess risk and perform crisis interventions.
- Ensure safety through tailored safety plans.
- Educate members about mental and physical health, abuse prevention, and medication.
