Coordinate afterhours patient care workflows and support on-call RNs, schedule patient visits using HCHB system, and review medical records. Process referrals and place patients in the appropriate care setting (home health, hospice, or palliative care). Educate patients and families about Gentiva’s services and next steps in the care journey, and maintain accurate documentation and patient tracking.
