Coordinate patient care from a distance using phone or video, focusing on assessments and care plans.
Identify process improvement through data collection and auditing.
Exhibit clinical excellence and ensure highest quality of care.

Qualifications:

Bachelors of Science in Nursing (BSN) required.
Minimum 3 years home health bedside experience; EPIC experience preferred.
Current Wisconsin RN license and BLS certification; OASIS certification preferred.

Work Environment:

Normally works in climate-controlled office environment with frequent sitting.
Frequent use of computers and keyboard; occasional exposure to communicable diseases.
Ability to lift up to 50 pounds and move freely.
