Renal Care Coordinator

About the Role:

• This is a full-time, remote position supporting nephrology practices.
• The Renal Care Coordinator provides telephonic care coordination for late-stage CKD patients.
• The goal is to help patients have an optimal start to dialysis.

What You'll Do:

• Assess patients' knowledge of late-stage CKD and educate on treatment options.
• Coordinate care and act as a liaison between patients and staff.
• Use motivational interviewing to engage patients and interpret clinical data.

What You'll Need:

• Minimum of 6 months experience as a Renal Care Coordinator with Interwell Health.
• 1-2 years of prior RCC experience is highly preferred.
• Strong organizational and communication skills and availability to work in practice time zones.
