Quality Provision of Services:
- Provide fully remote virtual serious illness care management to patients and families.
- Perform virtual assessments, identify and manage medications, and support the plan of care.
- Communicate and collaborate with the primary team to ensure continuity of care.
Organization/Regulatory Compliance:
- Maintain current knowledge of hospice and palliative medicine trends and regulatory standards.
- Obtain and maintain hospital/facility credentialing as required.
- Participate in Quality Assurance/Performance Improvement activities.
People/Communications:
- Communicate and collaborate with team members, patients, families, and partnering providers within HIPAA guidelines.
- Provide patient and family education to decrease anxiety and improve quality of care.
- Function as a role model in providing exceptional customer service through effective communication.
Financial Stewardship:
- Achieve expected productivity standards as defined by the organization.
- Implement time-based billing procedures using the electronic medical record system to capture timely billing.
