Virtual Care Coordinator

In this role, you’ll support patients and clinicians across the country by managing virtual patient check-ins, handling inbox messages, and ensuring seamless care coordination in a fully remote environment. This is an ideal opportunity for someone with prior virtual care experience who is comfortable navigating EMRs and multitasking in a fast-paced, digital-first clinic in a health tech startup environment. Responsibilities include conducting remote patient check-ins and pre-visit preparation including medication reconciliation, health history intake, and screening questionnaires. You will also document medical information accurately in the EMR and prep charts ahead of clinician visits. You will communicate with patients regarding upcoming appointments, test results, and follow-up instructions. You will coordinate care: manage referrals, prior authorizations, and medication refill requests. Ensure prompt and professional communication with patients, clinicians, and care team members.
