Manager of Chronic Care Management (CCM)

The Manager of Chronic Care Management (CCM) will oversee and manage the day-to-day operations of the CCM program, supervise and mentor the clinical staff, including pharmacists, nurses, and other healthcare providers. They will also develop and implement strategies to improve the CCM program, coordinate with interdisciplinary teams on patient care, and participate in tech development projects to streamline CCM processes. The manager will ensure adherence to regulatory guidelines and maintain up-to-date knowledge of developments in the field, while analyzing patient data to guide improvements in the CCM program.
