Bilingual Care Coordinator, LPN (Pacific Time)

Key Responsibilities:

• Perform monthly CCM touchpoints and build ongoing patient relationships.
• Conduct structured symptom assessments and evaluate changes using clinical judgment.
• Escalate urgent issues and coordinate care with providers and the care team.

Qualifications:

• Active LPN or LVN license and bilingual Spanish/English.
• Minimum 1-2 years clinical experience in ambulatory care, population health, or care coordination.
• Strong documentation skills and ability to work in a metric-driven remote environment.
