Clinical Intake Specialist

What you’ll do:

• Conduct outreach to newly referred clients and log activity in Salesforce.
• Conduct 8-12 intake visits daily, ensuring efficient and accurate assessments.
• Assist clients in completing initial intake visit including a psychosocial assessment and care plan with goals of care.

Your experience and qualifications:

• Active, unrestricted California Licensed Vocational Nurse (LVN) license, or Bachelor’s degree in a related field, or Medical Assistant certificate.
• Minimum of 2 years of previous intake or admissions experience in a high call volume environment.
• Experience working with complex care clients and ECM Populations of Focus.
