Accountabilities:
- Determine appropriate rehabilitation assessments to clarify needs and coordinate evaluations.
- Develop comprehensive Rehabilitation Plans that are needs-based, goal-oriented, and aligned with program mandates.
- Empower participants to identify solutions through community-based interventions and resources.
Requirements:
- Bachelor’s degree in a related field with a recognized health professional designation or CVRP credential.
- At least 3 years of experience in medical, psychosocial, or vocational rehabilitation services.
- Strong ability to develop plans, assess needs, and coordinate multidisciplinary services autonomously.
