Social Worker

Job Overview:

• Position is per diem and remote, providing palliative care duties.
• Part of an Interdisciplinary Team under Supportive Care Clinical Manager.

Key Responsibilities:

• Conduct psychosocial assessments and provide grief support.
• Screen for mental health concerns and safety risks.
• Participate in weekly IDT meetings and complete documentation within 48 hours.

Qualifications:

• Must have New Jersey LCSW, LMFT, or LPCC.
• 2+ years healthcare social work experience required.
• Experience in hospice, palliative care, oncology, or home health preferred.
