Per Diem Intake Nurse

What you will be doing:

• Review and clinically assess incoming referrals for home health services, ensuring compliance with Medicare and regulatory requirements.
• Collaborate with Clinical Managers and Intake Coordinators to coordinate admissions, prioritize referrals, and manage capacity.
• Communicate with physicians, hospitals, and referral sources to obtain documentation and facilitate timely admissions.

What we’re looking for in you:

• Current Massachusetts RN or LPN license with at least 3 years of nursing experience and 1+ year in home health preferred.
• Strong clinical assessment, critical thinking, and triage skills, with experience in Medicare and Managed Care.
• Excellent communication, organizational, and relationship-building abilities, with proficiency in electronic medical records.
