Role Responsibilities:

• Engage patients remotely to build relationships, assess needs, and provide education using motivational interviewing and various communication tools.
• Coordinate care by assisting with referrals, scheduling, and connecting patients to community resources to support health goals and preventive screenings.
• Document all interactions accurately in EHR systems and utilize analytics to support quality performance and risk adjustment initiatives.

Qualifications and Skills:

• Require an active Certified or Registered Medical Assistant credential with over 3 years of outpatient experience.
• Need strong critical thinking, communication skills, and knowledge of medical terminology and primary care workflows.
• Prefer bilingual abilities in Spanish and English, along with autonomy in a remote setting and customer-service orientation.
