About the Role:
- Provide person-centered care to individuals with serious illness, administering assessments and developing care plans.
- Lead care coordination as a key member of the multidisciplinary team with a technology-enabled approach.
Key Responsibilities:
- Administer and interpret assessments and screening tools to prioritize member needs.
- Develop individualized care plans and serve as the quarterback of the care team.
- Build strong, trusting relationships with members through listening and empathy.
