Care Coordinator (CST) - Bilingual, Spanish

The Role:

• Facilitate conversations and collaboration between interdisciplinary care teams, health plan care managers, and primary care providers.
• Support scheduling for Complex Care Navigators, make reminder calls, and ensure proper documentation of all patient contacts.
• Answer patient calls and appropriately transfer escalated issues while fostering patient relationships through empathy.
