Nurse Practitioner

The Role:

• Serve as the primary clinical point of contact for patients navigating their remote care journey.
• Lead clinical decision-making with a focus on guideline-directed medical therapy for heart failure and adherence to evidence-based guidelines for type 2 diabetes and hypertension.
• Collaborate closely with physicians, RNs, and Patient Success teams to deliver timely, patient-centered care.

What You'll Do:

• Manage a patient panel, making timely clinical decisions based on evidence-based guidelines.
• Initiate and titrate GDMT while collaborating with care teams to optimize outcomes.
• Respond to clinical escalations from remote monitoring, including abnormal vitals and lab results.

Compensation and Benefits:

• Competitive base salary of $130,000-$160,000 annually plus incentive compensation.
