Community Care Case Manager

Responsibilities:

• Conduct assessments and develop care plans for injured, acute, or chronically ill members to optimize medical outcomes and facilitate timely return to work or optimal functioning.
• Communicate with members, medical providers, attorneys, employers, and insurance carriers telephonically or in person, preparing all required case work documentation.

Work Arrangement & Requirements:

• Schedule is standard business hours Monday-Friday with no nights, weekends, or holidays; remote work requires a dedicated, interruption-free workspace.
• Requires an active RN license (Illinois licensure process must be initiated prior to start) and 3+ years of clinical experience, preferably with Medicare members and conditions like diabetes or CHF.

Qualifications & Application:

• Minimum 2+ years of case management, discharge planning, or home health care coordination experience; a Bachelor’s degree is preferred.
• Proficiency in MS Office and proprietary applications is required; multi-state RN licensure may be needed and will be provided by the company.
