This is a remote contractor position delivering RTM and CCM Services. You will converse with patients to ensure that they are improving every single day, facilitate patient enrollment, make daily calls and text messages to document patient health status and keep them engaged, and assist in he collection and documentation of patient health information.

You will play a crucial role in ensuring high-quality, patient-centered care across our telehealth operations. You'll conduct quality reviews, support Care Coach audits, and apply triage protocols. Collaboration between corporate and clinical teams will also be a key aspect of your role in shaping clinical standards and supporting the expansion of care delivery programs.
