LPN Care Coordinator

Role Overview:

• Salvo Health is seeking an LPN Care Coordinator to join the remote care team, focusing on chronic gut and metabolic conditions.
• The role involves providing continuous, app-based care and coordinating with a multidisciplinary team to deliver personalized patient journeys.

Key Responsibilities:

• Provide exceptional care, disease management, and health education to patients, supporting goal setting and behavior change.
• Conduct regular outreach and create personalized action plans to reduce unwanted behaviors and improve engagement.
• Coordinate with clinical team members and cross-functional product teams to enhance the patient experience.

Qualifications:

• Must hold a compact state license as an LPN or LVN with 2+ years of experience in patient-facing roles.
• Excellent communication skills, knowledge of medical terminology, and proficiency in digital tools like Slack and Google Workspace.
• Self-motivated with strong problem-solving skills; experience in telehealth or healthcare startup environment preferred.
