Key Responsibilities:
- Coordinate and oversee collaborative care for patients referred to Franciscan Health System under physician or ARNP direction.
- Guide patients through intake, diagnosis, treatment, and discharge, ensuring referrals and treatment plans reach appropriate specialists.
- Work with community services and government agencies to support ongoing medical needs.
Qualifications:
- Graduation from an accredited Practical Nursing program and two years of related clinical work experience.
- Must hold a Washington Licensed Practical Nurse (LPN) license and Basic Life Support (BLS) CPR certification.
- Must reside in the state of Washington.
Work Environment:
- This is a remote position with standard hours Monday through Friday, 8:00am to 5:00pm.
