About the Role:
- Owns the operational backbone of community-based care: the CHW workforce and Community Supports.
- Leads two teams through two leaders across twelve counties and eight health plan contracts.
What You Will Own:
- Deployment, hiring, and integration of community teams with clinical teams.
- Audit accountability, performance reporting, and unit economics for Community Supports.
