Key Responsibilities:

• Provide virtual care and clinical assessments for Medicare patients across multiple markets.
• Support initial assessments, care plan direction, and ongoing clinical oversight for complex cases.
• Manage chronic conditions, acute concerns, and medication questions through telemedicine.

What You'll Need:

• Active NP licenses in planned states like California, Texas, or New York and board certification as a Family or Adult-Gerontology Nurse Practitioner.
• Bilingual fluency in Spanish and English with 2+ years of clinical NP experience.
• Experience caring for Medicare, geriatric, or medically complex adult populations.
