Job Description

The Sr. Program Manager will lead a small team to identify new and execute existing opportunities driving total cost of care savings across Aledade’s ACOs. Primary duties include scoping and executing priority programs focused on interventions that improve patient health outcomes and manage total cost of care. The Program Manager will manage a team of program managers, cross-functional team members, and vendor relationships to effectively implement and measure the impact of approved programs. They will identify, scope, and evaluate new innovation opportunities by analyzing emerging trends in the industry, data, and input from Aledade SMEs and practices to identify potential opportunities. The Program Manager will size impact and cost of pursuing opportunities; define value proposition and case to pursue the opportunity and develop pilot proposals and implementation plans. This role also involves managing the process to document and disseminate best practices in program management and manage core team processes including synthesizing weekly updates and ‘savings pod’ meetings.
