The Patient Liaison is responsible for the intake of information from referral sources and the scheduling of patients, including the creation of initial files and obtaining necessary authorizations. They will also gather appropriate information needed to complete registration and provide appointments at suitable clinics for skilled therapy or functional testing.
This position is responsible for supporting the mission, vision, and values of Upstream Rehabilitation by ensuring patients receive timely scheduling that meets their medical needs.
This is a remote contact center environment with a requirement to work on the phone 100% of the time. This position does not have any supervisory duties.
