Palliative Care Transitions Specialist

About the Role:

• The Palliative Care Transitions Specialist manages patients through the palliative continuum, from referral and eligibility screening to timely transitions into hospice or other appropriate levels of care.
• This transitions-focused role identifies appropriate patients early and ensures goals-of-care conversations happen at the right time.

Key Responsibilities:

• Review incoming referrals, confirm eligibility, and tag cases appropriately in Zoho to prioritize urgent needs.
• Own transitions from palliative care to hospice, coordinating handoffs between providers and facilities.
• Complete care plans, document communications, and support monthly summaries and reports.

Qualifications & Fit:

• Candidates may have an LPN, RN, or Social Work background with direct clinical experience in hospice or palliative care.
• Strong organizational skills, phone presence, and proficiency with Microsoft Office and EMR systems are essential.
• Must be a positive, adaptable team player in a fast-paced environment.
