Role Overview:
- Supports Judi Health’s site-of-care optimization program by facilitating seamless transitions for infusion services.
- Serves as a key liaison between members, providers, and internal teams to ensure effective program execution.
Key Responsibilities:
- Conducts outreach and educates members on infusion therapy options, benefits, and resources.
- Manages case workloads, documentation, and escalations to maintain continuity of care.
- Collaborates with clinical, operational, and account teams to monitor performance and drive improvements.
Qualifications & Requirements:
- Minimum 3 years in a clinical setting, PBM, or health plan with medical case management experience.
- Proficiency in EMRs, Microsoft Office, and ability to manage multiple priorities independently.
- Reliable high-speed internet with wired connection and 1 Gbps minimum speed required.
