You Will:
- Support patients through enrollment, ongoing engagement, discharge, and step-down care.
- Conduct outreach to identify barriers and help patients navigate benefits, transportation, and community resources.
- Coordinate closely with psychiatrists, therapists, and external partners to ensure successful handoffs.
You Have:
- Bachelor's degree in social work, psychology, counseling, public health, or a related field.
- At least three years of behavioral health case management, care coordination, or discharge planning experience.
- Experience supporting higher-acuity populations and working with multidisciplinary clinical teams.
