Role:
- Coordinate care for home healthcare patients and serve as a liaison among patients, families, HHAs, and internal teams.
- Document and resolve cases in HHA Exchange, manage schedules, and complete daily, weekly, and monthly reports.
Requirements:
- Fluent in English and Spanish with previous HHA Exchange experience.
- Strong communication, customer service, and organizational skills.
Schedule:
- Monday–Thursday 9 AM–5 PM, Friday 9 AM–4 PM, with a 45-minute unpaid break.
- Must be flexible for weekend on-call support.
