What You'll Do:
- Engage families in person and by phone to reconnect children to care.
- Coordinate enrollment, onboarding, and community resources with the care team.
What You Bring:
- 3+ years in patient-facing healthcare; bilingual Spanish required.
- CHW certification preferred; valid driver's license and vehicle.
What We Offer:
- $23-$28/hour plus bonus, benefits, and 401(k) with 4% match.
