Referral and Authorization Care Coordinator

Key Responsibilities:

• Collaborate with a multidisciplinary team of care providers for whole-person care.
• Coordinate, track, and communicate patient referrals, authorizations, and medical records.
• Provide accurate registration of referrals and authorizations with patient demographics and insurance.

Qualifications:

• Minimum 2-3 years in healthcare coordination within a remote digital health space.
• Completed degree in health and science field.
• Fluent in healthcare terminology and navigating health insurance plans.

Compensation & Benefits:

• Salary range $45,000-$52,000 commensurate with experience.
