Qualifications:
- Bachelor’s degree in Social Work, Counseling, Psychology, Nursing, or related field.
- Licensed Social Worker (LSW), Licensed Independent Social Worker (LISW), Licensed Practical Nurse (LPN) or Registered Nurse (RN).
- Minimum 1 year of experience in home and community-based services within the last 5 years.
