The Role:
- Serve as a trusted frontline public health worker partnering with patients, families, and community organizations.
- Address social drivers of health, support chronic disease self-management, and connect patients to resources.
- Provide health system navigation, resource coordination, outreach, education, and coaching without clinical duties.
Qualifications:
- High school diploma or GED with experience in healthcare, public health, or community services.
- Knowledge of local resources and experience with Medicaid, rural populations, or underserved communities.
- Willing to complete a Michigan-recognized CHW training program; MiCHWA certification preferred.
