Care Coordinator - Remote Patient Services

Key Responsibilities:

• Coordinate patient enrollment, device setup, and provide education on remote monitoring technology.
• Oversee data collection, transmission, and storage, and analyze trends to identify health status changes.
• Collaborate with healthcare professionals to optimize patient care and use of monitoring technology.

Qualifications:

• At least 1 year of clinical work experience as a Medical Assistant.
• At least 1 year of EMR experience, preferably eClinicalWorks.
• High level of professionalism and excellent communication skills.
